  • Primary care is general health care. It is a patient's first point of contact for their health and wellness needs. Patients develop lifelong partnerships with their primary care provider (PCP). Your PCP takes ongoing responsibility for providing and managing your comprehensive health care.
  • Family medicine is comprehensive health care for your entire family - from newborns to your most senior members. All Trinity Health IHA Medical Group physicians are board-certified or board-eligible in family medicine. Our family medicine physicians deliver a wide range of acute, chronic and preventive medical care while providing you with a patient-centered medical home (PCMH). PCMH patients and their families (when appropriate) establish a partnership with their practitioners to ensure they have the support and education needed to make decisions and participate in their health care.
  • With a focus on preventive health and overall well-being, our internal medicine physicians and staff specialize in primary care for adults and patients with acute and chronic multi-system diseases.
  • Trinity Health IHA Medical Group offers nutrition counseling services at many practice locations. Nutrition counseling is provided by dedicated registered dietitians. Their expertise can provide you with the necessary knowledge to achieve all of your individual and family dietary needs.
  • At Trinity Health IHA Medical Group, care management works with patients who have moderate to complex illnesses. Located at your primary care practice, the care manager works in collaboration with your primary care physician and medical team to improve your overall health. The care manager works with you to assess and proactively manage your health, helping you to identify areas that you would like to improve or better manage. Together you will work to build skills and techniques to better manage your health care conditions. Your care manager will help you set realistic and obtainable goals, help you to identify solutions to overcome barriers and provide ongoing support and encouragement. This is a personalized service based on your needs. The care manager will call and meet with you regularly in your physician's office when it is convenient for you.
  • Are behavioral health challenges holding you back from the life you want to live? Issues such as depression, anxiety, addiction, ADHD, ongoing grief, relationship issues and insomnia are common and treatable. Trinity Health IHA Medical Group provides mental health services for adults, women, children, couples, families to help push past these common challenges. These services include psychiatric evaluation and medication management, outpatient therapy and psychological testing, and evaluation for ECT (electroconvulsive therapy). We help patients address the cause and learn how to make symptoms more manageable. You receive evaluations and treatments from mental health professionals who care about you as an individual. We use evidence-based practices and a personalized approach to help patients achieve their goals. With successful treatment, you can live the happy, productive life you’ve always wanted. Psychiatry and Counseling Trinity Health IHA Medical Group psychiatrists focus on the diagnosis, treatment, and prevention of mental, emotional, and behavioral disorders. Psychiatrists are medical doctors who specialize in assessing and addressing various mental health conditions, such as anxiety, depression and mood disorders. They utilize a combination of therapeutic interventions, including psychotherapy and medication management, to help individuals improve their mental well-being and overall quality of life.
  • Trinity Health IHA Medical Group ambulatory care clinical pharmacist services work with primary care providers as a part of the patient-centered medical home (PCMH). Through in-office and telephone appointments, they can help patients achieve optimal control of chronic conditions, such as diabetes, high blood pressure or high cholesterol. Clinical pharmacists provide comprehensive medication review services including, adjusting current medications to ensure maximum safety and efficacy, ongoing monitoring and disease state education. They also work with patients to set dietary and lifestyle goals to improve the management of various conditions.

About The Practice

Welcome to Trinity Health IHA Medical Group, Primary Care - Cherry Hill Village (formerly IHA Cherry Hill Village Primary Care).

Our providers and staff are dedicated to providing the highest quality medical care to our patients and the best service to their families. We offer daily appointments available for acute needs, and extended early and late hours several days per week, along with some Saturday hours, as well.

Trinity Health IHA Medical Group, Primary Care - Cherry Hill Village is a designated Patient-Centered Medical Home (PCMH). PCMH patients and their families (when appropriate) establish a partnership with their practitioners to ensure they have the support needed to make decisions and participate in their own health care.

As a practice, we know that preventive care, including vaccinations, is the most important part of our service to patients and their families. To best protect your children, our other patients, as well as our staff and providers from vaccine-preventable illness, we have made the decision to only accept families into our practice who plan to fully vaccinate their children according to the CDC, American Academy of Pediatrics and American Academy of Family Physicians guidelines.
