  • With a focus on preventive health and overall well-being, our internal medicine physicians and staff specialize in primary care for adults and patients with acute and chronic multi-system diseases.
  • Trinity Health IHA Medical Group offers nutrition counseling services at many practice locations. Nutrition counseling is provided by dedicated registered dietitians. Their expertise can provide you with the necessary knowledge to achieve all of your individual and family dietary needs.
  • Are behavioral health challenges holding you back from the life you want to live? Issues such as depression, anxiety, addiction, ADHD, ongoing grief, relationship issues and insomnia are common and treatable. Trinity Health IHA Medical Group provides mental health services for adults, women, children, couples, families to help push past these common challenges. These services include psychiatric evaluation and medication management, outpatient therapy and psychological testing, and evaluation for ECT (electroconvulsive therapy). Psychiatry and Counseling Trinity Health IHA Medical Group psychiatrists focus on the diagnosis, treatment, and prevention of mental, emotional, and behavioral disorders. Psychiatrists are medical doctors who specialize in assessing and addressing various mental health conditions, such as anxiety, depression and mood disorders. They utilize a combination of therapeutic interventions, including psychotherapy and medication management, to help individuals improve their mental well-being and overall quality of life.
  • Pediatrics is a specialty concerned with the physical, mental and social health of children from birth to young adulthood. Pediatric care encompasses a broad spectrum of health services ranging from preventive health care to the diagnosis and treatment of acute or chronic diseases. A pediatrician can accurately determine the child’s health status and serve as a consultant and make use of other specialists when needed. Because the child’s welfare is very dependent on the home and family, the pediatrician supports efforts to create a nurturing environment.   • Trinity Health IHA Medical Group ambulatory care clinical pharmacist services work with primary care providers as a part of the patient-centered medical home (PCMH). Through in-office and telephone appointments, they can help patients achieve optimal control of chronic conditions, such as diabetes, high blood pressure or high cholesterol. Clinical pharmacists provide comprehensive medication review services including, adjusting current medications to ensure maximum safety and efficacy, ongoing monitoring and disease state education. They also work with patients to set dietary and lifestyle goals to improve the management of various conditions.

About The Practice

Welcome to Trinity Health IHA Medical Group, Primary Care - Plymouth (formerly IHA Plymouth Primary Care).

We are a group committed to providing quality medical care to our patients. We offer same-day sick appointments, some late appointments and more. Two of our physicians are internal medicine/pediatrics providers, which means they can see children as well as adults.

Along with our regular hours, we offer extended early and late hours several days a week.

Trinity Health IHA Medical Group, Primary Care - Plymouth is a designated Patient-Centered Medical Home (PCMH). PCMH patients and their families (when appropriate) establish a partnership with their practitioners to ensure they have the support needed to make decisions and participate in their own health care.

As a practice, we know that preventive care, including vaccinations, is the most important part of our service to patients and their families. To best protect your children, our other patients, as well as our staff and providers from vaccine-preventable illness, we have made the decision to only accept families into our practice who plan to fully vaccinate their children according to the CDC, American Academy of Pediatrics and American Academy of Family Physicians guidelines.
