- Family medicine is comprehensive health care for your entire family - from newborns to your most senior members. All Trinity Health IHA Medical Group physicians are board-certified or board-eligible in family medicine. Our family medicine physicians deliver a wide range of acute, chronic and preventive medical care while providing you with a patient-centered medical home (PCMH). PCMH patients and their families (when appropriate) establish a partnership with their practitioners to ensure they have the support and education needed to make decisions and participate in their health care.
- Primary care is general health care. It is a patient's first point of contact for their health and wellness needs. Patients develop lifelong partnerships with their primary care provider (PCP). Your PCP takes ongoing responsibility for providing and managing your comprehensive health care.
- The Trinity Health IHA Medical Group Collaborative Care Behavioral Health Program works with patients who may have anxiety, depression or both. This program is available at several of our locations. We are right here in your office! Our team consists of a behavioral health specialist, your primary care physician (PCP) and a psychiatrist who talks with your behavioral health specialist and PCP. We work together to help improve your overall well-being. About once a month, the behavioral health specialist will call you or meet you in the doctor’s office to check in, ask about your mood symptoms and they may discuss how your medication(s) are working. We use questionnaires to help track changes in your symptoms. If you are part of this program, the behavioral health specialist works with you to build skills and techniques to manage your depression or anxiety. If you want to use medication to help control your symptoms, our team psychiatrist will talk to your behavioral health specialist and your PCP to determine the best treatment options.
- Trinity Health IHA Medical Group ambulatory care clinical pharmacist services work with primary care providers as a part of the patient-centered medical home (PCMH). Through in-office and telephone appointments, they can help patients achieve optimal control of chronic conditions, such as diabetes, high blood pressure or high cholesterol. Clinical pharmacists provide comprehensive medication review services including, adjusting current medications to ensure maximum safety and efficacy, ongoing monitoring and disease state education. They also work with patients to set dietary and lifestyle goals to improve the management of various conditions.
- Sports Medicine providers practicing non-surgical sports medicine are highly trained in treating the musculoskeletal system, including, bones, muscles, joints, tendons, ligaments, and connective tissue. They treat injuries for athletes and non-athletes without operating on the body. Sports Medicine – Non-Surgical providers can diagnose, treat, and manage injuries and underlying conditions that impact the patient’s physical performance. They also offer guidance on when it’s safe for patients to return to normal activity after an injury like, sprains, muscle strains, fractures or breaks, joint injuries, overuse injuries, tendinitis, osteoarthritis, or concussions. Non-surgical sports medicine providers perform minor office-based procedures such as cortisone or hyaluronic acid injections when indicated for conditions such as arthritis or tendinitis. In addition to treating injuries, non-surgical sports medicine providers can help patients who want to improve their activity level or performance or need guidance on injury prevention, safe ways to exercise and build strength.

As a practice, we know that preventive care, including vaccinations, is the most important part of our service to patients and their families. To best protect your children, our other patients, as well as our staff and providers from vaccine-preventable illness, we have made the decision to only accept families into our practice who plan to fully vaccinate their children according to the CDC, American Academy of Pediatrics and American Academy of Family Physicians guidelines.
