- Family medicine is comprehensive health care for your entire family - from newborns to your most senior members. All Trinity Health IHA Medical Group physicians are board-certified or board-eligible in family medicine. Our family medicine physicians deliver a wide range of acute, chronic and preventive medical care while providing you with a patient-centered medical home (PCMH). PCMH patients and their families (when appropriate) establish a partnership with their practitioners to ensure they have the support and education needed to make decisions and participate in their health care.
- With a focus on preventive health and overall well-being, our internal medicine physicians and staff specialize in primary care for adults and patients with acute and chronic multi-system diseases.
- Primary care is general health care. It is a patient's first point of contact for their health and wellness needs. Patients develop lifelong partnerships with their primary care provider (PCP). Your PCP takes ongoing responsibility for providing and managing your comprehensive health care.
- Trinity Health IHA Medical Group ambulatory care clinical pharmacist services work with primary care providers as a part of the patient-centered medical home (PCMH). Through in-office and telephone appointments, they can help patients achieve optimal control of chronic conditions, such as diabetes, high blood pressure or high cholesterol. Clinical pharmacists provide comprehensive medication review services including, adjusting current medications to ensure maximum safety and efficacy, ongoing monitoring and disease state education. They also work with patients to set dietary and lifestyle goals to improve the management of various conditions.
- Trinity Health IHA Medical Group offers nutrition counseling services at many practice locations. Nutrition counseling is provided by dedicated registered dietitians. Their expertise can provide you with the necessary knowledge to achieve all of your individual and family dietary needs.
- Are behavioral health challenges holding you back from the life you want to live? Issues such as depression, anxiety, addiction, ADHD, ongoing grief, relationship issues and insomnia are common and treatable. Trinity Health IHA Medical Group provides mental health services for adults, women, children, couples, families to help push past these common challenges. These services include psychiatric evaluation and medication management, outpatient therapy and psychological testing, and evaluation for ECT (electroconvulsive therapy). We help patients address the cause and learn how to make symptoms more manageable. You receive evaluations and treatments from mental health professionals who care about you as an individual. We use evidence-based practices and a personalized approach to help patients achieve their goals. With successful treatment, you can live the happy, productive life you’ve always wanted. Psychiatry and Counseling Trinity Health IHA Medical Group psychiatrists focus on the diagnosis, treatment, and prevention of mental, emotional, and behavioral disorders. Psychiatrists are medical doctors who specialize in assessing and addressing various mental health conditions, such as anxiety, depression and mood disorders. They utilize a combination of therapeutic interventions, including psychotherapy and medication management, to help individuals improve their mental well-being and overall quality of life.

• Use office resources and our extensive health care network to provide the highest quality diagnostic and therapeutic techniques for our patients.
• Exercise belief in preventive health by educating patients about healthy lifestyles, immunization and maintaining well-being.
If you have an urgent medical problem and need assistance when the office is closed, please call the office and your call will be directed to the Trinity IHA Call Center. The call center staff will assist you and contact the physician on-call to address your concerns.
We encourage you to sign up for the Trinity Health MyChart patient portal, which offers patients personalized and secure online access to portions of their medical records.

As a practice, we know that preventive care, including vaccinations, is the most important part of our service to patients and their families. To best protect your children, our other patients, as well as our staff and providers from vaccine-preventable illness, we have made the decision to only accept families into our practice who plan to fully vaccinate their children according to the CDC, American Academy of Pediatrics and American Academy of Family Physicians guidelines.
