  • Our Specialty
    Services
  • Primary care is general healthcare. It is a patient's first point of contact for their health needs. Patients develop lifelong partnerships with their Primary Care Provider (PCP). Your PCP takes ongoing responsibility for providing and managing comprehensive healthcare.
  • Family medicine is comprehensive health care for your entire family - from newborns to your most senior members. All our physicians are board certified or board-eligible in family medicine. Our family medicine physicians deliver a range of acute, chronic and preventive medical care while providing you with a Patient Centered Medical Home (PCMH). PCMH patients and their family (when appropriate) establish a partnership with their practitioners to ensure they have the support and education needed to make decisions and participate in their own health care.
  • At IHA, Care Management works with patients who have moderate to complex illnesses. Located in your primary care practice, the care manager works in collaboration with your primary care physician and medical team to improve your overall health. The care manager works with you to assess and proactively manage your health, helping you to identify areas that you would like to improve or better manage. Together you will work to build skills and techniques to better manage your health care conditions. Your care manager will help you set obtainable goals that are realistic, help you to identify solutions to overcome barriers and provide ongoing support and encouragement. This is a personalized service based on your needs. The care manager will call and meet with you regularly in your physician's office when it is convenient for you.

About The Practice

Welcome to IHA South Lyon Primary Care (formerly IHA Family Medicine - South Lyon)!

IHA South Lyon Primary Care is a practice dedicated to providing quality medical care to our patients and life-long care for all of your family. From your newborn to your most senior members - the whole family can receive personal, quality care from our licensed, board-certified family physicians. Our goals are to find illness early, following both national and local guidelines for screening, use office resources and our extensive health care network to provide the highest quality diagnostic and therapeutic techniques for our patients, and exercise belief in preventive health by educating you about healthy lifestyles, immunization, and maintaining well-being.

If you have an urgent medical problem and need assistance when the office is closed, please call the office and your call will be directed to the IHA Call Center. The call center staff will assist you and contact the physician on-call to address your concerns.

We encourage you to sign up for the IHA Patient Portal, which offers patients personalized and secure online access to portions of their medical records.

IHA South Lyon Primary Care has been designated a Patient Centered Medical Home (PCMH). PCMH patients and their family (when appropriate) establish a partnership with their practitioners to ensure they have the support needed to make decisions and participate in their own health care.

As a practice we know that preventive care, including vaccinations, is the most important part of our service to our patients and families. To best protect your children, our other patients, as well as our staff and providers from vaccine-preventable illness, we have made the decision to only accept families into our practice who plan to fully vaccinate their children according to the CDC, American Academy of Pediatrics and American Academy of Family Physicians guidelines.
